LYME SYMPTOM WORKSHEET

Before seeing your physician, I recommend that you print and complete the Lyme Symptom Worksheet so that you have a comprehensive document detailing your symptoms and their severity.  I also recommend that you make a copy and give your doctor a copy to keep for his records. 


Due to the fact that Lyme symptoms tend to come and go, each symptom has three letters:  

B = BEFORE

Symptom occurred or present BEFORE any possible bite and are unrelated to Lyme 

P = PAST

Symptom experienced in the PAST but is not currently an issue 

N = NOW 

Symptom is being experienced NOW or recently. 

GENERAL

SKIN 
B  P N  Reddening usually of the palms, soles, or upper cheeks. 
B  P N  Pinna and ear lobes have varying shades of red. 
B  P N  A benign nodule or tumor on ear lobe, nipple, scrotum (Lymphocytoma) 
B  P N  Progressive dermatitis of hardened erythematous plaques that eventually give a tissue-paper appearance to the affected areas (Acrodermatitis Chronica Atrophicans) 
B  P N  Sensation of water dripping on skin or tingling sensations (like an insect crawling on skin) 
B  P N  Cysts or Polycystic Kidney or Fibrocystic Breast Disease. 
B  P N  Nodules in peripheral nerve sheath. 
B  P N  Eczema, hives, blotches, patches,welts, sun burn like, or poison ivy like rash and on dark skins may resemble a bruise and reappearance of “Bull’s eye” rash from time to time. 
B  P N  Burning pain like; sunburn/freshly scraped skin embedded with sand. 
B  P N  Spots or areas on the body that suddenly become extremely sensitive to touch.

SLEEP

DIGESTIVE SYSTEM 
B  P N  Disturbances of tongue,gag reflex, and swallowing. (Numbness)

Frequent urgent need to urinate with difficulty to suppress. Loss of bladder awareness. Emptying instantly upon movement. Urine leakage. Sudden inability to contract muscles.

JOINTS AND MUSCLES
B  P N  A sudden bout of arthritis with swelling, redness, and pain in one or a few 
large joints, typically the knees, that persists, or no longer persists, that may have left the affected joints distorted.

B  P N  Creaking, popping, or cracking joints 
B  P N  Joint stiffness 
B  P N  Bone sensitivity 
B  P N  Sudden agonizing bone pain of an entire length of a bone, the entire rib cage, or all bones connected to any single bone, sometimes too sore to touch.
B  P N  Sudden "axe-like/broken bone" type pain in a bone, especially near a joint when asleep or in awake time also. 
B  P N  Tenderness in lower shin medial tibia shaft bone. 
B  P N  Foot, ankle, and heel pain Mimics plantar fasciitis 
B  P N  Deep aches and burning in palms/soles of feet. 
B  P N  Muscle cramps and spasms (all over body) 
B  P N  Throughout body; Muscle tenderness and pains of varying intensity as if over-stretched, bruised, cut, scraped, injured, burned, sharp, dull, or aching. Mimics Fibro myalgia, including tender points that come and go. 
B  P N  Tendon and ligament pain. 
B  P N  Tingling, numbness, weakness, or pain felt in the hand, forearm, or wrist upon awakening from sleep. Arm(s) inflamed and swollen, for no reason.Mimics  Carpal Tunnel Syndrome (tennis elbow) 
B  P N  Muscle weakness/Body weakness 
B  P N  In babies; Low muscle tone 
B  P N  Unexplained backaches. Deep and steady pain in spine that feels as if it radiates from pressure on the nerves surrounding it or as if the pain comes from deep inside of the spine. (Any place on length of spine) 
B  P N  Intense pain, mostly in the lumbar and cervical regions radiating to the extremities, or disk disturbances.

NEUROLOGICAL ISSUES 
B  P N  Abnormal brain waves, seizures, mini blackouts, periods of confused memory, episodes of staring, unexplained periods of unresponsiveness. 
B  P N  Fainting spells 
B  P N  Abnormalities of taste or smell (Peculiar metallic or salty taste). 
B  P N  Visual, odor or auditory hallucinations 
B  P N  Tremors, unexplained shaking, jerking, twitching. 
B  P N  Shocks, sharp jabbing, and shooting pains anywhere in the body 
B  P N  Sensations in the body of pinpricks, stabbing, burning. 
B  P N  Sensations of vibration or flutters. 
B  P N  A sensation of wearing an invisible "glove" or "sock" 
B  P N  Numbness in parts of the body and/or extremities. 
B  P N  Increased motion sickness 
B  P N  Difficulty walking or moving the arms because of high level of pain. 
B  P N  Dizziness, poor balance, weakness, or partial paralysis of limbs, loss or diminished reflexes or coordination. 
B  P N  Feet/legs experience poor automatic response; Tripping, walking into things, clumsiness, not knowing where your feet are. 
B  P N  Burning, freezing, throbbing, shooting pain in limbs, hands and feet or  toes and fingers or all, that is often worse at night and is felt equally on both sides periodically or constant. Lyme Peripheral  Neuropathy 
B  P N  Involuntary movement of arms, legs, and digits. 
B  P N  Irresistible urge to move legs; sensations include: creeping, itching, pulling, creepy-crawly, tugging, or gnawing in legs. Restless legs syndrome

MENTAL AND EMOTIONAL 
B  P N  Feeling you are losing your mind/going crazy because the people around you disbelieve the reality of your pain, disabilities, and deteriorating health. 
B  P N  Suicidal thoughts for relief from physical/emotional pain and/or suddenly suicidal. 
B  P N  Unusual sadness, hopelessness, guilt, or depression. 
B  P N  Mood swings. 
B  P N  Over-emotional reactions to fit the situations. 
B  P N  Easily frustrated, irritated, agitated, short tempered. 
B  P N  Sudden explosive irritability for no apparent cause. 
B  P N  Aggressiveness. Lyme rage. 
B  P N  A lot of activity or noise within environment triggers extreme agitation. 
B  P N  Sudden moments of suspiciousness/paranoia. 
B  P N  Trying to do everything during periods of feeling okay and total inactivity when laden with pain. 
B  P N  Periods of extreme high energy to extreme lows. Mimics Bipolar Disorder. 
B  P N  A driving need to do something repeatedly and having much anxiety if unable to do it. 
B  P N  Persistent thoughts that cause anxiety/distress. 
B  P N  Experiencing anxiety most of the time, for no reason. 
B  P N  Panic attacks. 
B  P N  Feeling detached from ones self or reality. Feeling of “unrealness” 
B  P N  Always hurried, even without a deadline. 
B  P N  (Students) Uncooperative, defiant, negative. Rebellious and annoying behaviors.

COGNITIVE AND MEMORY ISSUES 
B  P N  Brain fog; Like a cloud reducing your visibility/clarity of mind.

B  P N  Difficulty in thinking and processing information.

B  P N  Interrupting others in mid conversation or contributing to the conversation long after it is finished. 
B  P N  Difficulty in expressing ones self. Difficulty in word finding. Speech comes out as stammering, slurred, slow, hesitating. 
B  P N  Emotional and expressive incontinence 
B  P N  Slow processing, recalling, or remembering of information or what was read. 
B  P N  Difficulty with basics, social, and day to day functioning. 
B  P N  Declining performance in school or work. 
B  P N  Feeling a significant decline in intellectual acuity 
B  P N  Feeling a loss  of competence. 
B  P N  Child who is misbehaving in class. 
B  P N  Intermittent distorted memory. Short/long term memory lapses. Suddenly forgetting how to perform a task, but remembering later. Mimics Alzheimer Disease 
B  P N  Tendency to get geographically disorientated; suddenly forgetting where a familiar destination is. Getting lost. Going to the wrong place. Going past the right place (as in severe sudden absentmindedness.)

HEAD, FACE AND NECK
B  P N  Inflamed brain; fever, headache, nausea and phobophobia. Lyme Encephalitis 
B  P N  Inflammation of the membranes/fluid just beneath skull surrounding the brain and  tissues surrounding spinal cord.  Headache, flu-like malaise. Aseptic meningitis 
B  P N  Long-lasting,(weeks to months) persistent/severe/ head pressure /congestion /burning/inflammation of the meninges layers covering the brain.(Chronic meningitis} 
B  P N  Burning in the brain, neck, and spinal cord as if inflamed. 
B  P N  Swollen forehead or edema in forehead. 
B  P N  Stiff or painful neck. Lightheadedness, wooziness, feeling off balance 
B  P N  Numbness usually only on one side off the face Lyme's Bell's palsy 
B  P N  Facial neuralgia's: Twitching of facial muscles (tics), sensation of sharp facial pain, crawly feeling, or itching,  tingling of nose and other facial parts. 
B  P N  Sensation of a spider web or loose hair fallen on face. 
B  P N  Drippy nose (unexplained) Mimics sinus disease 
B  P N  Sore throat, chronic cryptic tonsillitis, clearing throat, phlegm, hoarseness. 
B  P N  Cracks around sides of mouth. Dry mouth 
B  P N  Facial flushing (red face) 
B  P N  Loss or altered sense of smell/hearing/taste.

EARS AND HEARING ISSUES  
B  P N  High pitched whining, buzzing, clicking, ringing, buzzing, hissing, humming,  ticking, whistling, roaring, beeping or a "whooshing" sound, as of wind or waves, or other sound in one or both ears.

B  P N  Pain in ear(s) with no medical cause. 
B  P N  Sound sensitivity (Some noises causes extreme irritability)
